New Patient Forms

All new patients are required to fill out forms which enable our practice to efficiently address patient correspondence, billing and financial services. Please be advised if you have not been here within the last three years you must complete these forms. Please complete the new patient forms and bring them with you.

If you have any laboratory testing or radiology scans that you wish to review with the doctor please bring a copy of the results and/or scans to your appointment.

At your first consultation appointment with the gastroenterologist your medical history, symptoms and referral from your GP will be evaluated and further tests or procedures will be arranged for you. You will usually be examined by the gastroenterologist with reference to your symptoms.

Your medical file is handled with the utmost respect for your privacy. Our staff are bound by strict confidentiality requirements as a condition of employment regarding your medical records. Ordinarily we will not release the contents of your medical file without your consent.

Routine colonoscopy to look for early signs of cancer should begin at age 50 for most people—earlier if there is a family history of colorectal cancer, a personal history of inflammatory bowel disease, or other risk factors. The doctor can advise patients about how often to get a colonoscopy.
